In-person & tele appointments
When you click the book appointment link it will take you to my Headway page. Headway is the company I've partnered with to handle credentialing, scheduling and billing. They will email you new patient paperwork, please have this filled out and returned to Headway 3-5 days before your appointment, they need this time to process it so I can see you.
